Map & Directions to Hamden Health Care Center
Traveling North or South on I-91 Take Exit 10, stay on this connector to the end. At traffic light, bear right. At 3rd traffic light, go left on to Sherman Avenue. Travel to the end of Sherman Avenue, approximately 2-3 miles. At the stop sign, you're looking at the facility. Traveling North on the Wilbur Cross Parkway (RT 15) Take Exit 61 (Whitney Avenue/ Hamden). Go right off the exit on to Whitney Avenue traveling north. Go approximately 1/3 mile, take a left on to School Street. Go through traffic light, continue straight up Hamden Hills Road. 3/4 mile Hamden Health Care Center is on left. See cupola on roof. Traveling South on the Wilbur Cross Parkway (RT 15) Take Exit 62 (Whitney Avenue/ Hamden). Take right off ramp and follow directions as above. |
